PART A:
*Required for all families
Re-enrollment for the student information section will now be completed via the PowerSchool Portal. Parents must log into their PowerSchool Parent Portal to complete their student’s registration. If you do not have an account, you will be required to create one.

PART B:
* Required for scholarship students.
If your student receives a scholarship, please complete Part B. Due to the NEW requirements by the Ohio of Department of Education, original signatures on paperwork are now required.

2026-2027 Outreach - Autism Scholarship Application Packet
Part B includes completing all of the following:
1) Completing and signing the scholarship application and additional Outreach documents
2) Submitting a current proof of residency in your name *Click here for acceptable forms of address proof
3) Current immunizations or provide a Refuse to Vaccinate form
Final step to submit your completed application packet:
Once you have completed Part B (the application packet), please submit it along with your current proof of residency and other required forms though one of the following methods:
Scan/email: Scan/email all completed forms to Destiny.Stevens@newstoryschools.com
USPS Mail: Mail all completed forms to New Story Schools Ohio, 7649 New Market Center Way, Columbus, OH 43235 Attn: Outreach Re-Enrollment
In Person: Come in person and drop all completed forms off at the nearest school office

ENROLL NOW AND GET ENTERED IN A $500 RAFFLE!
EARLY BIRD RAFFLE:
By completing your student’s re-enrollment by the May 1st deadline, you will be automatically entered to win a $500 gift card. Please be aware, if your student’s re-enrollment is received after May 1st, they will be placed on a waitlist and could possibly lose their seat. So don’t delay and turn the re-enrollment paperwork in ASAP!
As a reminder, a fully completed re-enrollment includes both Part A AND Part B to be eligible to win in any of the raffles.
